Job Description:

This postiion is in the Social Work service with the Home and Community Care Team at the Hampton VA Medical Center. The Social Work Home and Community Care program specialist is an integral member of the Interdisciplinary Management Team as needed in the delivery of care for patients receiving Non-Institutional Care services from the Hampton VA Medical Center.
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 02/10/2025. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-9 position you must have served 52 weeks at the GS-7. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. You may qualify based on your experience and/or education as described below: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS 7 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Planning, scheduling, and conducting projects and studies to evaluate and recommend ways to improve the effectiveness and efficiency of work operations in a program or support setting; developing new or modified work methods, organizational structures, records and files, management processes, staffing patterns, procedures for administering program services, guidelines and procedures, and automating work processes for the conduct of administrative support functions or program operation; knowledgeable in applying analytical and evaluative techniques to the identification, consideration, and resolution of issues or problems of a procedural or factual nature; using qualitative and quantitative analytical techniques such as: work measurement; task analysis and job structuring; productivity charting; development and administration of questionnaires; flowcharting of work processes . OR, Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have possess a master's or equivalent graduate degree OR two (2) full years of progressively higher level graduate education leading to a master's or equivalent graduate degree . OR, Combination: Applicants may also combine education and experience to qualify at this level. You must have an combination of specialized experience and education beyond . Physical Requirements: The work is primarily sedentary. Typically, the employee sits comfortably to do the work. There is some walking, standing, bending, and carrying required. Duties Include: Conducts studies, analyzes and evaluates the needs of HCC Non-Institutional Care and devises specific solutions to meet the needs of the service. Reviews data and performs studies, summarizes information, identifies trends and proposes and implements solutions. Knowledge of the principles, practices, procedures, and techniques for conducting systematic reviews of data collection management, and analysis in a health services research setting; ability to write concise summaries of findings via publication and/or presentation. Requires in-depth knowledge of technical and administrative principles and techniques to coordinate the activities and services of the various components. Responsible for participating in coordination of initiatives, performance goals and development and implementation of strategies and actions related to the initiatives. Assists the service in analyzing situations, presenting and offering solutions to administrative issues, and coordinating special projects and programs that require integration of various individuals and teams throughout the Hampton VAMC. Acts as patient advocate to resolves problems, expedite services, or implement necessary corrective measures within established facility policies; assists individuals in preparing required documents, or in resolving errors, delays or other problems in obtaining benefits or fulfilling obligations. Specialized knowledge of the operating budget/supply/contracting practices, computerized staffing methodologies, space management and health care delivery process. Knowledge of computer operations including word processing, spreadsheets, graphing, database management and presentation software sufficient to note consults reports and patient wait times.
