Job Description:

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
To qualify for this position, you must meet the basic requirements as well as any additional requirements (if applicable) listed in the job announcement. Applicants pending the completion of training or license requirements may be referred and tentatively selected but may not be hired until all requirements are met. Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), b) OR [(2) Those approved by the American Osteopathic Association (AOA),OR (3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences. Residents currently enrolled in ACGME/AOA accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as "Physician Resident Providers" (PRPs). PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee-basis. PRPs are not considered independent practitioners and will not be privileged; rather, they are to have a "scope of practice" that allows them to perform certain restricted duties under supervision. Additionally, surgery residents in gap years may also be appointed as PRPs. Proficiency in spoken and written English. Additional Requirement: Board certification in internal or family medicine is required unless excluded by VH A directive. And specialty board eligible for geriatric and or palliative medicine sub specialty. Preferred Experience: Experience in extended care, long term care, home care and or Hospice programs. Experience in quality and performance improvement. Experience with interdisciplinary teams. Physical Requirements: Applicants must meet physical standards for the position. A physical examination prior to placement is required. This is a designated drug testing position. After appointment, you will be subject to random testing for illegal drug use. Requires lifting 15-44 pounds; pushing (approx. 2 hours); reaching above shoulder; use of fingers and both hands; walking and standing from 3-5 hours and kneeling. Ability for rapid mental and muscular coordination simultaneously. Must have depth perception and ability to distinguish basic colors and shades of colors. Hearing aid is permitted.

The GEC physician will have the following direct clinical responsibilities: Serves as a primary care provider for a panel of veterans for medical evaluation and treatment and by request from other team members when clinically indicated and collaborating With RN and inter-professional team for patient case management needs. Works with an inter-professional team to deliver primary care to veterans In their place of living. Includes being available during the tour of duty for phone, video, and face to face visits., conducting solo and/or joint home visits with team members as needed, and providing leave coverage for other providers as needed. Participates in CLC on call rotation for after hours coverage from home. Formulates and implements appropriate non-pharmacological and pharmacological Plan of care. Integrates Veteran's goals of care and collaborates with inter-professional team, Veteran, and Veteran Support System in formulating and implementing plan of care as appropriate. Provide education to veterans and their support systems as needed and appropriate. Determine the need for and initiate referrals and consultation in accordance with the standard of practice and or specified protocols. Obtain consent for procedures, transfers, or opioid contracts, as appropriate. Completes documentation and enters orders in Veteran's health record in compliance with local, regional and national standards and guidelines. Meet minimum clinical productivity expectations as determined by the service line and health system. Administrative support responsibilities: Precepting and collaborating with the nurse practitioners. Providing backup ACOS in their absence. Being familiar with VA and local policies that may affect patient care programs. Quality and performance improvement support responsibilities: Supporting ongoing survey readiness in the CLC, including participation in VA internal surveys and periodic performance review. Working with CLC leadership and staff on quality and performance improvement projects. Participating in peer review activities and. Proctoring as required by VA Sierra Nevada healthcare system policies. Reviewing and evaluating incident reports and making recommendations to the CLC leadership and the Sierra Nevada Healthcare System Administration as needed. Advising the ACOS GEC on the appropriateness of scope of services and the adequacy of professional and support staff and medical equipment. Medical Center responsibilities: Work with the CLC leadership to articulate the mission and vision of the CLC within and across service lines and to assist with strategic planning. Promote the mission of CLC by serving as a liaison between leadership. Other departments and services, the teaching and training program and GEC. Educate and update staff, providers, and leadership across the Medical Center about operational and patient care issues specific to CLC. Serve as the VA Sierra Nevada healthcare system medical staff, attend meetings as needed and uphold the medical staff bylaws, rules and regulations. Serve and or appoint designees to serve on hospital committees pertinent to GEC. Serve on university committees such as geriatrics and gerontology curriculum committees.
