This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Preferred Experience: Specialized experience with Serious Mental Illness (SMI) population. Experience facilitating groups is required. Case management experience preferred. Skills related to completing intake assessments, treatment plans, and discharge plans preferred. Three years of behavioral health experience preferred. The Bath VAMC is hiring a Social Worker for the Psychosocial Rehabilitation and Recovery Center (PRRC) and Mental Health Intensive Case Management (MHICM) programs. The Social Worker provides clinical services for both the PRRC and MHICM programs. Primary responsibilities of this position are providing case management, individual therapy, group counseling, family counseling, and clinical assessments to program veterans. This position works with the Serious Mental Illness (SMI) population. Major Duties:-Independently performs case finding functions and outreach, identifies and screens patients for psychosocial needs. PRRC/MHICM Program Social Worker must possess the knowledge and ability to independently implement treatment modalities and provide counseling or psychotherapy for individuals, families and groups at the appropriate level of licensure and within current scope of practice-Provide intensive case management services to assigned veterans and significant others, consistent with the goals and methods as outlined for each program, Program Evaluation Center (NEPEC) and with all relevant Medical Center policies and procedures. Frequent, direct contacts with patients to provide therapy and support. Frequency of contacts is increased in times of crisis. Flexibility of veteran contact (veterans are seen in any location and with any team clinician who can provide assistance). Provision of continuity of care within the framework established by each program. Outreach and assertive provision of services to treatment resistant patients.
