This Pulmonary Rehab Nurse Practitioner is located in the Medical Service at the Jamaica Plain campus location. This position is part-time at 28 hours per week. Advanced Practice Nurse (Nurse Practitioner) Basic Requirements Education: A post-master's certificate or master's or doctoral degree as a Nurse Practitioner. (Applicants pending the completion of educational requirements may be referred and tentatively selected but may not be hired until the education requirements are met.) Must be a graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). NOTE: Foreign Education. Graduates of foreign schools of professional nursing programs, the nurse practitioner degree equivalency must be validated through an Agency recognized degree equivalency evaluation agency, such as the International Consultants of Delaware (ICD). Candidates with equivalent nurse practitioner masters or doctorate level degrees who possess current, full, active, and unrestricted registration and certification will meet the requirement of graduation from an approved school of professional nursing to warrant appointment. License and Certification: License. All APN (NPs) must possess and maintain an active, current, full, and unrestricted APN license to practice as an NP in a State, Territory or Commonwealth of the United States, or in the District of Columbia. Basic Certification. All APN (NPs) must maintain full and current board certification in an area of clinical specialty from one of the following: the American Nurses Credentialing Center (ANCC), American Academy of Nurse Practitioners Certification Board (AANPCB), or other nationally recognized certifying body in the area in which the Nurse Practitioner is academically prepared. Accepted Certifications - Family NP, Adult-Gerontology Primary Care NP. NOTE: Grandfathering Provision - All persons currently employed in VHA in 0610 series and an APN (NP) assignment that are also performing the duties as described in the qualification standard on the effective date of the standard (1/29/2024) are considered to have met all qualification requirements for the grade held including positive education and licensure/certification. The following Scope, Education and Experience, and Dimension criteria must be met to qualify for Advanced Practice Nurse (Nurse Practitioner) II: Exception to Licensure for Graduate Nurse Technicians. Candidates who otherwise meet the basic requirements, but do not possess the required licensure and/or certification, may be appointed at the entry step of the Nurse II, as a Graduate Nurse Technician - Nurse Practitioner on a temporary appointment up to any period not-to-exceed one year and a day under the authority of 38 U.S.C. § 405(c)(2)(B). The Pulmonary Rehab APN (PRAPN) provides comprehensive wellness care as well as acute, episodic, and chronic care of adult clients participating in the Pulmonary Rehab Program. The PRAPN is prepared by advanced practice in adult health care across the health continuum. This practice includes independent and interdependent decision-making and direct accountability for clinical judgment. The position(s) fall under the Pulmonary Medicine Section of the Medical Service with interaction and supervision by Pulmonary staff physicians. Duties include, but are not limited to: Maintains accreditation of Pulmonary Rehab Program by the American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR). Work collaboratively with a multi-disciplinary team to provide supervised exercise training and disease self-management to patients with chronic lung disease. Interact with primary care, pulmonary, pulmonary transplant, and other services related to the care of patients enrolled in the pulmonary rehabilitation program. Manage all consultations made to the Pulmonary Rehabilitation Program. Supervise exercise classes (in-person and virtual) to ensure safety of patients during exercise training. Perform initial assessments of medical history, health-related quality of life, and psychosocial assessments. Follow-up of laboratory evaluation, perform resting and exercise oximetry, and perform EKG if needed. Perform exercise evaluation with 6-minute walk tests and assess for patient safety. Formulate an exercise treatment plan and outcome evaluation and plan with supervising MDs. Oversee tasks related to delivering educational component of pulmonary rehabilitation. Interact with nutritional services to formulate nutritional plan and weight management plan. Assess vital signs and perform finger sticks as needed at baseline and during exercise. Adjust treatments for exercise-induced alterations in blood pressure, blood sugar, or bronchospasm during exercise program. Assess oxygen requirements and perform titration of supplemental oxygen with respiratory therapist. Perform follow-up evaluations and services to patients completing the program. Complete transition back to primary care providers related to post-rehab exercise maintenance program. Lead management of day-to-day maintenance of the exercise gym. Manage data from patient enrollment, testing and questionnaires to continuously monitor performance and quality of pulmonary rehabilitation program. Perform duties related to internal and external oversight and accreditation. Supervise the virtual telehealth pulmonary rehabilitation classes and contribute to growth of future pulmonary rehabilitation sites.
