Job Description:
The Clinical Pharmacist at Tomah Veterans Affairs Medical Center (VAMC) position functions in the clinical sections of the Pharmacy service line under the Chief of Staff's Office. The primary functions will be the interpretation and verification of medication orders, Veteran medication profile review (allergy, drug interaction screens, etc.) providing drug and policy information to Veterans and medical center staff, and participation in educational programs within the service and the VAMC.

Basic Requirements: Citizen of the United States. (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.) Education Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Graduates of foreign pharmacy degree programs meet the educational requirement if able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. (Must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.) English Language Proficiency - must be proficient in spoken and written English Grade Determinations: In addition to the basic requirements the following criteria must be met: Experience. One (1) full year of creditable experience at the next lower level (GS-11) or equivalent, directly related to the Clinical Pharmacy position which handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. These include, but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution. Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs): Knowledge of professional pharmacy practice. Ability to communicate orally and in writing to both patients and health care staff. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. A Clinical Pharmacy is responsible for consulting with medical center staff when there are questions concerning therapeutic or policy matters regarding outpatient prescriptions or inpatient physician orders. Verifies accuracy and appropriateness of medication orders and prescriptions during order finishing. When appropriate, fills outpatient prescriptions and inpatient medication orders and issues ward or clinic orders for drugs, supplies, and narcotics as necessary. Maintains proper records for all transactions. Ensures formulary guidelines are followed in accordance with all applicable rules, regulations, and guidelines. Duties include, but not limited to: Clinical Functions The position assures the accuracy of drug dispensing and the compounding of medications based upon Veteran's history and medication profile review, good clinical practice, and other pertinent factors. Provides drug information to physicians, nurses, and other associated health professionals. Provides oral and written drug information to inpatients and outpatients, as appropriate. Participates in Pharmacy and medical center quality assurance activities and the medical center's Medication Use Evaluation (MUE) program, as appropriate. Assists the Chief of Pharmacy in maintenance of the formulary. Gathers information supporting the inclusion of drugs into (or removal of drugs from) the formulary at the national, Veterans Integrated Service Network (VISN), or local level. Educational Functions Assists with instruction of professional or technical level Pharmacy students and assists in the training of new Pharmacy Technicians and Pharmacists. Participates in educational programs for the medical staff and drug educational activities including (but not limited to) participation in new provider and new nurse orientation. Participates in patient education throughout the medical center. Maintains knowledge and completes continuing education in therapeutic areas of expertise. Administrative Functions Communication and interpersonal relations, including the ability to appropriately and courteously relate to internal and external customers. Maintenance of confidentiality of patient/employee information, electronic and print. How to work and function in the medical center in a safe manner, including infection control and the correct operation of equipment. Interpersonal Relations Functions Maintains excellent working relationship with department and organization staff. Courteous and professional at all times. Effectively promotes teamwork among staff, residents and students. Provides prompt and personal attention to each patient. Demonstrates responsiveness to customer needs and is empathetic to their problems. Performs other duties as assigned by management.
