This is a Direct Hire Solicitation About the Position: This position is responsible for the clinical pharmacy functions of the Patient Centered Medical Home (PCMH). Serves as an expert on the delivery of pharmaceutical care, which involves an extensive knowledge of pharmacology, pharmacokinetics, pathophysiology, pharmacotherapeutics and their interplay in treating human disease.

Basic Requirement for Pharmacist (Clinical): Education: A doctoral degree in Pharmacy that is recognized by the Accreditation Council for Pharmacy Education (ACPE) or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Exemption: You are exempt from the basic requirement above if you are a current federal employee occupying a position in the 0660 occupational series and have been continuously employed in this occupational series since September 27, 2017, or before. Note: You will be required to provide appropriate documentation to the respective Human Resources Office to validate your status. Licensure: A current, active, valid, and unrestricted license to practice pharmacy in a State, a territory of the United States, or the District of Columbia. Medical Requirement: Must be able to distinguish basic colors. Experience: One year of professional pharmacy experience which includes demonstrated experience managing medication-related information in electronic health records; analyzing patient specific data to evaluate current drug therapy; dispensing and administering medications as needed in a clinical setting; AND conducting patient and family centered education related to medication treatment plan.
Analyzes patterns and trends to determine appropriate, effective and economical prescribing and dispensing of medications.
Develops in coordination with expert physician consultants, prescribing guidelines for a number of drug categories or Diagnosis Related Groups.
Reviews existing drug treatment for appropriateness and develops with the attending physician new treatment regimens for especially difficult or complex cases.
Monitors progress and response to the regimen, modifying dosages as necessary.
