Job Description:
The incumbent serves as a Social Work Program Coordinator, locally titled Caregiver Support Program (CSP) Manager, which provides a state-of-the-art continuum of services, education and resources to caregivers of Veterans with complex needs. The incumbent oversees service delivery and coordination of care activities for family caregivers who provide personal care services to eligible Veterans regardless of period of service.
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for master's degree in social work. Licensure: Candidate must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the basic requirements, applicants must have at least one year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. In addition to the experience above, candidates must demonstrate all of the following KSAs: Skill in coordinating and implementing one or more specialty program(s), across the facility. Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to collaborate with internal and external partners to further program goals and enhance patient centered care. Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action. Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget. Establishes and maintains an ongoing education program for Veterans, caregivers, community agencies, students, and staff to facilitate understanding of clinical practice guidelines, mental health issues, readjustment issues and psychosocial issues facing caregivers. Develops and facilitates CSP-program focused direction, orientation, coaching, in-service training, staff development, and continuing education programs for VA facility staff and the community. Develops and implements a plan for outreach to Veterans and Veteran caregivers who may benefit from the CSP program services (but who do not presently use VA for their health care needs). Maintains a current network of referral resources to include peer support mentoring groups, caregiver skill building and self-care educational opportunities, mental health support, legal and financial services. Supervises the VA HCS Caregiver Support Coordinator(s) (CSC) and, when applicable, Program Support Assistant (PSA) and other interprofessional staff assigned to Caregiver Support Program as outlined in VHA Directive 11152(1), Caregiver Support Program. Coordinates program implementation with multiple clinical services across all campus locations within the VA facility, including but not limited to nursing; physical, occupational, and speech therapists; mental health and primary care providers. The incumbent may provide clinical interventions to the Veteran and caregiver populations served by the programs. Serves as a liaison to Veterans Integrated Service Network Program (VISN) CSP Program and the VA Central Office (VACO) Program. Works collaboratively with facility, VISN, and VA Central Office staff to develop and implement solutions participating in national, regional, and VISN operational calls; contributing to national leadership council committees; and implementing cultural transformation initiatives (e.g., Elizabeth Dole Foundation's Campaign for Inclusive Care (CIC), the Rosalyn Carter Institute). Performs the administrative and personnel management function relative to staff supervised including personnel actions, clinical supervision, consultation, negotiation with other departments, and quality improvement. Works collaboratively with internal staff, VISN and VACO leadership, and external stakeholders to expedite service delivery. The incumbent is responsible for monitoring performance measures at the local, VISN, and Central Office levels; providing responses to congressional inquiries; and briefings to providers, VA leadership, community organizations and other internal and external stakeholders. Oversees the daily operations of the CSP program. Develops policies and procedures for program operation, and prepares reports and statistics for facility, VISN, and national use. Manages CSP clinical and business operations, accountability for program effectiveness, and modification of service patterns to comply with the delivery of standardized processes found within Directive 1152(1). The incumbent supports customer satisfaction, well-being and service delivery. Addresses Congressional inquiries, VISN, and VA Central Office action items as requested. Develops a data-driven, continuous quality improvement program with established goals and outcomes to evaluate and document the program's effectiveness. Manages all aspects of the PFCAFC and the PGCSS to ensure eligible caregivers have awareness and access to the national Caregiver Support Line (CSL), VHA's Peer Support Mentoring Program, self-care and resiliency building trainings (e.g. Building Better Caregivers), and personal care service skill development trainings (e.g., VHA Caregiver Core Curriculum) regardless of era of service. Develops a robust PGCSS that not only administers the required programing and services outlined within policy and regulation (i.e., education, training, and technical support; access to VHA services of Telehealth and Respite Care; and mental health counseling) but also actively promotes national VHA resources (e.g., Peer Support Mentoring Program; Caregiver Support Line; National Family Caregiver Month (NFCM) events) on a consistent basis.
