The Palliative Care Social Worker (Program Coordinator) is located within the Social Work Service of the Bay Pines VA Healthcare System. Incumbent will provide services at C.W. Bill Young Medical Center. The duties and responsibilities are carried throughout the HealthCare Center including all clinical and other patient care areas involved with the services.

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates. GS-12 Social Worker (Program Coordinator) candidates must have the following: Experience and Education: One (1) year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. The primary responsibility of the incumbent is to provide leadership, guidance, and direction for the clinical and administrative findings of the Palliative Care Program. Administers information and analytical systems to evaluate and enhance the quality of service provided to patients. Develops and implements information systems to track service activities including visits, referrals, inpatient days, patient demographics, equipment utilization and patient satisfaction. Develops implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services. Duties and responsibilities include, but are not limited to: Performs work related to the review, monitoring, analysis, planning and assessment of the Palliative Care program. Collaborates with community partners to streamline and maintain the most appropriate level of care to meet patient needs. Responsible for key project activities related to the planning, development, and implementation of the project component. The incumbent performs a wide range of services and provides technical assistance to State, local, and private agencies, and universities. Technical assistance provided is based on monitoring activities and program reviews, and the development and analysis of information. In addition, the incumbent uses sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements. The incumbent performs program analysis, development, review, research, design, and contact work as possible with various agencies (public, DOD, state, etc.). The incumbent ensures adequate program coverage and makes changes accordingly. The incumbent ensures that work is completed in a timely manner and reflects quality documentation in accordance with Service, Medical Center, VA and The Joint Commission policy and standards.
