The VA Eastern Colorado Health Care System (ECHCS) is a leader for best practices for the Care Coordination & Integrated Case Management (CC&ICM) program. ECHCS has a strong geriatric program with diverse routes for aging Veterans, providing collaborative clinical services including eConsults, telephone, Clinical Video Telehealth, and VA Video Connect (VVC). The workload may be carried out at one or more locations that are physically removed from the main unit.
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Profiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. ยง 7403(f). Education: Possess a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the 4 accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). NOTE: If social workers covered under the grandfathering provision of the 1991 Federal law regarding licensure or certification of VHA social workers leave the GS-0185 social work series, they lose the grandfathering protection. If they choose to return at a later date to the GS-0185 series, they must be licensed or certified to qualify for employment as a social worker. Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates: Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. Total Rewards of a Allied Health Professional As the Social Worker Program Coordinator the incumbent is administratively responsible for a clinical program. Provides treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program Other duties may include but are not limited to: Oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. Provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service Assists in planning, establishing, and implementing policies and procedures. Implementing a new or revised process, developing individual or group goals and objectives as well as monitoring, operating, evaluating, coordinating the assigned program. Responsible for ensuring compliance with the associated VHA Directives, field guidebooks, and recommended practices. Closely collaborate with internal stakeholders of the specialized and complex /programs, creating processes and procedures which support the delivery of services across the care continuum. Function as the subject matter expert in geriatric social work and across ECHCS Performs clinical duties that involve specialized treatment of complex physical and/or mental illness. Incorporates complex multiple causation in differential diagnosis and treatment of veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. Maintains knowledge of benefits and services relating to special programs, service-connected compensation, and non-service-connected pension. Must initiate and grow relationships with partner agencies and work in partnership with clinical staff. Adhere to all current directives of VHA, and meet current Joint Commission standards, State licensure standards, and VA-OIG expectations.
