Job Description:

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants are ineligible.
Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Tect (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Grade Determinations: GS-12 The following criteria must be met when determining the grade of candidates. Experience: In addition to the basic requirements, candidates must have 1 year of experience equivalent to the next lower grade level (GS-11) and demonstrate the following KSAs: 1. Knowledge of professional pharmacy practice. 2. Ability to communicate orally and in writing to both patients and health care staff. 3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. 4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). The actual grade at which an applicant may be selected for this vacancy is 12. Physical Requirements: Applicants must be able to perform the following duties: lifting (up to 50 lbs); carrying (up to 50 pounds). Applicants must have use of all fingers. Walking or standing for up to 8-hours. Kneeling intermittently multiple times for up to 8-hours. Use of both legs is required. Must possess visual acuity to read typed print. Keen hearing (aids permitted). Mental stability must be required to handle rigors of the position. The loss or impairment of a specific function may be compensated by satisfactory use of prosthesis.
VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws The incumbent is responsible for analyzing real and potential drug-related medical problems and implementing corrective action to ensure that patients receive optimal drug therapy. Evaluate of the appropriateness of drug therapy based on patient specific factors. Provide individualization of drug therapy. Evaluation, dispensing and providing medications, and drug information. Demonstrate the knowledge and skills necessary to complete the duties of the position, including the necessary skills to provide care appropriate to the population of the patients served. Function in the Healthcare System in a safe manner, including infection control and the correct operation of equipment. Reviews charts, evaluates pertinent laboratory data, drug-drug and drug nutrient interactions, monitors for adverse drug effects, and screens for allergies. Performs medication reconciliation at all transitions of care as appropriate, including updating the medication profile to reflect an accurate, active list of VA and non-VA medications. This may include adding non-VA medications or discontinuing duplicate medications or those the patient is not taking. Assesses drug safety and efficacy, including evaluation of physical symptoms. Participates in the healthcare system's medication utilization evaluation (MUE) program as assigned. Recommends formulary alternatives as assigned. Implements Pharmacy & Therapeutics Committee policies including therapeutic interchanges, renal dosing, IV to PO, and automatic substitutions to include appropriate patient and provider education. Manages various actions, as appropriate, for medication orders to include flagging prescriptions, managing view alerts, using the "hold medication" function, and generating unsigned, discontinued orders using Service Reject. Responsible for maintaining his/her professional competency by keeping abreast of current medical and pharmaceutical literature, new drugs and therapies, and applies this knowledge in his/her daily duties. Appropriate documentation of all clinical activities in the electronic health record and ancillary systems, as designated, in a timely manner. Performs other duties as assigned by supervisor. Reviews all medication orders for appropriateness, drug selection, dosage, route of administration and the amount, prior to dispensing. Ensures all medication orders are entered into the electronic patient medical record. Reviews patient's allergy history, drug-drug, drug-nutrient, drug-laboratory and drug-disease state interactions and assessment for clinical relevance should be performed and dealt with appropriately. Reviews medication profiles and monitors for compliance or potential abuse; problems encountered shall be resolved.
