This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. Grade Determinations: Clinical Pharmacist GS-11 Experience, Education, and Licensure. None beyond the basic requirements. Clinical Pharmacist GS-12 Experience or Education. In addition to the basic requirements, candidates must meet one of the following: 1 year of experience equivalent to the next lower grade level, or Completion of an ACPE-accredited Pharm.D. program. Assignments: A pharmacist in this assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. These include, but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution. Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs): Knowledge of professional pharmacy practice. Ability to communicate orally and in writing to both patients and health care staff. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: VA experience, PGY-1 trained a plus. Extensive Outpatient Pharmacy experience preferred. Physical Requirements: The work involves long periods of moving, standing and pushing carts about the work unit. The work requires regular and recurring bending, stretching, and lifting. The clinical pharmacist is a licensed professional with knowledge and proficiency in pharmacy practice including pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics. The incumbent is responsible for analyzing real and potential drug-related medical problems and implementing corrective action to ensure that patients receive optimal drug therapy. The clinical pharmacist also applies standards relating to all aspects of distribution and control of scheduled and non-scheduled drugs. The primary function of a clinical pharmacist is to assure the safe and appropriate use of medications and be an advocate of rational drug therapy through the following: evaluation of the appropriateness of drug therapy based on patient specific factors, individualization of drug therapy, evaluation, dispensing and providing medications, and drug information. Duties and responsibilities include but are not limited to: The outpatient clinical pharmacist supports the general operations of Pharmacy including prescription/order processing, outpatient dispensing, patient consultation, and perform all pharmacy related activities as assigned and on any shift. The pharmacist provides appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; and advises prescribers as appropriate. Provides patient-specific therapeutic drug monitoring and communicates relevant findings and/or recommendations to other health care providers in charge of the patient both orally and/or in writing. Monitoring will include perspective review and intervention in: (a) Therapeutic appropriateness of a patient's drug regimen (b) Therapeutic duplication in the patient's drug regimen (c) Appropriateness of the route and method of administration (d) Degree of patient compliance with the prescribed drug regimen (e) Drug-drug, drug-food, drug-laboratory, drug disease interactions, allergies, appropriateness of drug and dose within the context of the individual patients' medical conditions. (f) Clinical and pharmacokinetic laboratory data to evaluate the efficacy of drug therapy and to anticipate side effects, toxicity, or adverse effects (g) Physical signs and clinical symptoms relevant to the patient's drug therapy Maintains records as required by law. Serves as a drug information resource by providing up-to-date drug information to prescribers, other health care professionals, patients, and caregivers. Reports Adverse Drug Events (ADE), near misses, and medication errors and alignment with VA ADERS reporting program. Recommends formulary alternatives and completes non-formulary and restricted medication consult as assigned. Manages various actions, as appropriate, for medication orders to include flagging prescriptions, managing view alerts, and generating unassigned discontinued orders using service reject. Maintains competence required for current position including required TMS training, and BLS certification. Properly operates all equipment in the assigned area(s) in an efficient manner and assist with troubleshooting automated dispensing equipment (ADE), other assigned equipment's or technology platforms/databases. Maintains adequate drug stock, inspects drug storage areas, supervises pre- packing and the operation of automated dispensing equipment. The pharmacist is responsible for maintaining his/her professional competency by keeping abreast of current medical and pharmaceutical literature, new drugs, and therapies, and applies this knowledge in his/her daily duties. May be assigned to assist with duties or other work areas in pharmacy service to help fulfill our mission and abide by our organization's values.
