Job Description:

The Clinical Pharmacy Specialist (CPS) at VA Central California Health Care System performs at the highest level of clinical practice and works under a scope of practice as approved by the PSB/Medical Staff Executive Committee. The CPS functions to initiate, modify, discontinue and monitor medication therapy and acts as a consultant for medication therapy management service

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. Pharmacists must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, Appendix G15. Education. (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. All applicants must possess a full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. Grade Determinations: In addition to the basic requirements for employment above, the following criteria must be met when determining the grade of candidates. Clinical Pharmacist Specialist GS-13 Experience: In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level Clinical Pharmacy Specialist. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Pharmacists assigned to this position must demonstrate the following KSAs: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). The full performance level of this vacancy is GS -13. Physical Requirements: Physical aspects associated with work required of this assignment are typical of the occupation. May be required to wear personal protective equipment to undergo annual TB screening or testing as conditions of employment. The incumbent must satisfactorily complete a pre-employment physical to include moderate lifting (15-44 pounds), light carrying 15 lbs or less, straight pulling, reaching above the shoulder with the use of both hands and fingers, ability to distinguish basic colors and shades of colors, which is required for this position.
NOTE: One of the positions will provide services for the Merced Outpatient Clinic. The selectee for this vacancy will be required to travel to Merced Clinic. The CPS oversees pharmacotherapy to ensure that national treatment guidelines are being followed in addition to adherence to national formulary and local polices. Interfaces with providers, including physicians, PA, ARNPs in the design and continued evaluation of the Veteran's treatment regimen to achieve optimization of rational and cost-effective drug therapy . Serves as an authoritative information source on medications and their utilization in therapy. Evaluates drug literature by analysis of experimental design and methodology, in order to compare and contrast therapeutic regimens and roles for new drugs. Recognizes patient presentation of signs, symptoms, laboratory tests and diagnoses, recognizes and list important medical problems, disease states, symptoms or abnormal laboratory values that may necessitate altering the therapeutic regimen or which may be induced by drug therapy. Establishes and expands the patient data base through interpreting the drug history, physical examination and ongoing patient specific monitoring of therapy, with documentation in the formal patient medical record. Monitors, assesses and documents therapeutic and adverse effects of drug therapy (including drug interactions) through selection and evaluation of physical and laboratory parameters. Provides physicians and other providers a resource for discussion of medical problems requiring a pharmaceutical intervention. Interviews each patient conducting a medication history and formulating a pharmaceutical care plan. Makes recommendations to the provider for optimal management of drug-induced problems either verbally or in writing. Provides medication reconciliation at all appropriate patient encounters and transitions of care. Orders, performs, reviews and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy. Provides patient care utilizing an advanced in-depth knowledge of a specialized area of clinical pharmacy practice. Educates patients on the proper use and care of their medications and related medical devices through the day-to-day personal interactions with the patients. Provides formal education and training programs in the area of pharmacology, therapeutics, pharmacokinetics and related subjects to providers, nursing staff and other health care professionals as appropriate. Serves as a mentor/preceptor to Pharmacy students and residents. Plans and coordinates formal and informal instruction/orientation to all stall within area of assignment. Serves as a Pharmacotherapy expert and mentor to medical students, residents and physicians extenders, teaching them pharmacotherapy and healthcare economics. Use appropriate references to research drug information. Provides accurate and timely responses to drug information inquires from all customers. Serves as an authority on medication usage, interactions, over-dosages and compliance to clinical staff, residents and students.
