The VA Eastern Colorado Healthcare System,, Social Work Service is currently recruiting for one (1) Medical Foster Home and Veteran Directed Care Social Worker Program Coordinator, GS-0185-12, at the Department of Veterans Affairs Clinic, in Colorado Springs or Aurora, Colorado.
To qualify for this position, applicants must meet all requirements by the closing date of this announcement. Basic Requirements: United States Citizenship. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. ยง 7403(f). Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/. Grade Determinations: Social Worker (Program Coordinator), GS-12 Experience and Education. One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Individual may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social work program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. In the performance of duties, the incumbent may be required to drive and/or ride in GSA vehicles.
Total Rewards of a Allied Health Professional Major duties and responsibilities of the position include but are not limited to: Develops and implements information systems to track service activities including visits, referrals, assessments, patient demographics, equipment utilization, and patient satisfaction. Performs a wide range of services and provides technical assistance to State, local, and private agencies, and universities. Technical assistance provided is based on monitoring activities and program reviews, and the development and analysis of information. Performs work related to the review, monitoring, analysis, planning, and assessment of Medical Foster Home(MFH) and Veteran Directed Care (VDC). Responsible for key project activities related to the planning, development, and implementation of MFH and VDC. . Serves Veterans who tend to have serious, frequent and severe crises, lack family or an adequate community support network, be poor at self-monitoring; frequently fail to comply with instructions and treatments; have significant deficits in coping skills and require continuing psychosocial and medical support. The incumbent incorporates complex multiple causation in differential diagnosis and treatment of Veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. Responsible for maximizing psychosocial quality of life issues in order to assist Veterans to remain in their home or in a less restrictive level of care other than a nursing home and keep them out of the hospital as often as possible. May provide individual, group, and family psychotherapy and advanced level case management interventions used in the treatment of Veterans with polytrauma injuries, spinal cord injuries, traumatic brain injuries, visual impairment, post-traumatic stress disorder, addictions, and other serious disorder. Oversees program development, recruitment, and development of the foster homes, recruitment and placement of Veteran residents, case management, development of policies and care processes, and coordination with Home Based Primary Care (HBPC) interdisciplinary treatment teams. Responsible for the selection of homes and residents; establishing intake processes, establishing oversight responsibilities, and maintaining a record-keeping system for the program. Serves as a mentor to other VA MFH program coordinators, social workers, and other interested health care providers. Provides training to caregivers, students, residents, interns, and fellows from other disciplines regarding the advantages and disadvantages of MFH. Topics include but are not limited to caregiver and family issues and the impact of psychosocial issues and problems of the Veteran in a medical foster home. On occasion, the incumbent may be required to transport Veterans, in a government vehicle, as a part of the treatment plan, to visit potential medical foster homes. Screen VDC referrals to ensure appropriateness and assessment is completed prior to entering the program with emphasis on Veteran's strengths, limitations, supports systems, and service needs. Ongoing program assessment: Work with the Veteran's medical team and the VDC agency to monitor the Veteran's level of functioning, completing re-assessments as dictated by the program's Field Guidebook. Monitor the Veteran's budget and spending. Complete all community care consults for VDC and will refer to and coordinate with the agency of jurisdiction. Collaborate with care providers and VDC staff to assist in case management, establish appropriate goals and make referrals based on the Veteran's preference.
