About the Position: This position is within the Behavioral Health/Mental Health Directorate located at Naval Hospital Twenty Nine Palms, California. This is a Direct Hire Solicitation. A recruitment or relocation incentive may be authorized. Salary negotiation may be available for those candidates who are new to Federal service. Non-Federal work experience may be creditable for leave purposes.

Basic Requirement for Behavioral Health Case Management: Degree: Bachelor's or graduate (or higher level) degree with major study in an academic field related to the medical field, health sciences or allied sciences appropriate to the work of the position. This degree must be from an educational program accredited by an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below: Specialized Experience: One year of specialized experience which includes conducting behavioral health assessments/interviews; coordinating behavioral health care plans with a multidisciplinary team; and evaluating appropriateness, quality, or cost effectiveness of care. This definition of specialized experience is typical of work performed at the second lower grade/level position in the federal service (GS-11). Some federal jobs allow you to substitute your education for the required experience in order to qualify. For this job, you must meet the qualification requirement using experience alone--no substitution of education for experience is permitted.
Serve as a behavioral health case manager on a multidisciplinary medical team, conducting complex assessments and coordinating care for high-risk patients.
Develop and implement individualized treatment plans, monitor medication effects, and provide psychoeducation, skills training, and therapeutic interventions.
Facilitate care transitions and collaborate with internal and external providers to ensure continuity and quality of care.
Lead weekly treatment planning meetings, analyze patient data to identify trends, and recommend program improvements.
Analyze patient data to uncover trends, guide program improvements, and support smooth transitions between levels of care.
