JOB DEFINITION: The Program Manager assumes overall daily responsibility for the Health Home Care Management (HHCM) program to ensure that clients assigned to the program receive client-centered, integrated, multidisciplinary coordinated care that promotes better health and more appropriate utilization of health care.
